We want clients to understand our scheduling, payment, treatment, and patient-care policies before their visit. Please review the information below carefully. If you have questions about how a policy applies to your pet or appointment, contact us before your scheduled visit.
Appointments & Scheduling Fees
A scheduling fee is required when making an appointment:
- $60 for standard appointments
- $45 for recheck appointments
- $25 for technician appointments
Scheduling fees are credited to your account and applied toward the total cost of the visit.
Scheduling fees for general appointments are non-refundable and non-transferable within 48 hours of the scheduled appointment. Missed appointments, as well as appointments canceled or rescheduled within 48 hours, will forfeit the scheduling fee. A new scheduling fee will be required to make another appointment.
Appointments are scheduled in 20-minute increments. Arriving late may result in an extended wait, additional charges, or the need to reschedule. Please arrive on time for your scheduled appointment.
Surgery Scheduling & Requirements
A $100 surgery scheduling fee is required to schedule any surgery. This fee is credited toward the total cost of the surgery but is non-refundable and non-transferable under all circumstances.
All surgical patients must be current on a physical exam performed by Dr. Rivera and on all age-appropriate vaccinations administered by a licensed veterinarian. If proof of required vaccinations has not been provided by the scheduled surgery time, the appointment will be forfeited and the scheduling fee lost.
Pre-anesthetic bloodwork is required for any sedation or anesthetic procedure in patients over one year of age or for patients with certain medical conditions that may affect anesthesia.
Patient & Appointment Requirements
The legal owner and/or guardian, age 18 or older, must be present for each patient’s initial office visit. At that time, other adults age 18 or older who may be legally and financially responsible for the pet will be added to the account for future visits.
A physical exam is required at least once yearly to maintain a current veterinarian-client-patient relationship. Pets taking medications may require bloodwork or more frequent examinations in order to continue receiving refills.
Any new or recurring medical problem requires a physical exam to establish a new baseline for treatment.
All canine and feline patients must be current on their rabies vaccination. If current records are not provided, the required vaccination will be administered at the appointment. Clients who refuse a necessary rabies vaccination will be reported to the appropriate local authorities.
All rabies vaccinations require a physical exam by Dr. Rivera at the time they are administered.
Medical Records
Clients are responsible for providing their pet’s medical history and records. Records must be sent before the scheduled appointment.

Payment & Financing
Payment is due in full at the time services are rendered.
Utah Veterinary Hospital does not offer in-house financing. Payment plans may be available through Cherry for fully approved estimates of $500 or more. We also accept CareCredit.

Checks are not accepted.
We cannot accept payment from a third party. If you are collecting donations toward a needed procedure for your pet, you are responsible for collecting those funds and paying the bill in full when your pet is discharged.
Financing approval, terms, payment amounts, and eligibility are determined by the financing provider.
Treatment Plans, Refunds & Medications
Utah Veterinary Hospital provides a tailored treatment plan for each patient outlining planned treatment and associated costs.
No refunds are offered after services have been administered.
Medications cannot be returned once they leave the hospital. There are no exceptions to this policy.
Prescriptions & Refills
Pets receiving ongoing medications may require bloodwork or more frequent examinations to continue receiving refills.
Medications filled through an outside pharmacy require a written prescription. A minimum $5.50 fee per medication applies to prescriptions written for outside pharmacies.
Once the prescription has been provided, Utah Veterinary Hospital is not responsible for the actions of the outside pharmacy.
Restraint, Safety & Client Conduct
All patients must be properly restrained with a well-fitted collar or harness and leash, or secured in an appropriate carrier.
Clients accept responsibility for their property, their pet, and their actions while at Utah Veterinary Hospital. By entering the hospital, clients acknowledge that inherent hazards and risks may be present in a veterinary setting.
Utah Veterinary Hospital does not tolerate rude, abusive, or disrespectful behavior. Clients who display this behavior will have their veterinarian-client-patient relationship terminated and will no longer be welcome at the clinic.
